Important Forms

All students traveling abroad need to turn in the following documents to the Center for Professional Excellence at least one month before they leave the U.S.

NOTICE! YOU MUST FILE AN APPLICATION WITH THE OFFICE OF INTERNATIONAL EDUCATION IN THE CENTER FOR PROFESSIONAL EXCELLENCE FOR ALL LONG-TERM STUDY ABROAD PROGRAMS!

Required for All Programs

• Release and Authorization Form
• Code of Conduct Form (38k pdf)
• Student Information Form (25k pdf)
• Clear copy of passport
• International Student Identification Card (ISIC). 

[Note: You may purchase an ISIC at the Center for Professional Excellence for $22.00 after turning in all the documents above. You’ll need to bring a 1” x 1.5” photo of yourself.]

Faculty Forms and Requirements

• Faculty/Staff Information Form
• Medical Report Form for Faculty/Staff
• Faculty Release and Authorization Form
• Detailed itinerary
• Clear copy of passport
• International Teacher Identification Card (ITIC).

[Note: You can receive your ITIC at the Center for Professional Excellence.  You must provide a 1" x 1.5" photo of yourself.]
